PURPOSE

The purpose of this policy is to outline the process of responding to requests post recovery from transplant
centers for additional tissues (including spleen, nodes, and vessels) and/or blood samples from a University
of Wisconsin Organ and Tissue Donation (UW OTD) organ donor. UW OTD will ensure that the organ donor
tissue and/or blood sample being sent will be properly labeled, safely packaged, and sent in a timely
manner to the requesting transplant center.

POLICY

A. Written requests for additional tissue and/or blood samples will be provided to UW OTD by a
transplant center via fax or email.
B. The request will include the following information:
1. Specimen requested
2. United Network for Organ Sharing (UNOS) identification number of the donor
3. Donor name
4. Donor date of birth.
C. The request will be processed by UW OTD staff.
D. Transportation arrangements for the sample will be negotiated between UW OTD and the
requesting transplant center so that the package will arrive in a safe and timely fashion.
E. UW OTD staff will label and package tissues for transportation in accordance with United
Network for Organ Sharing (UNOS) policy 16.0
F. UW OTD staff or their designee will label, package, and ship blood specimens according to the
shipping specification of the requesting transplant center.
G. UW OTD will file a copy of the written request in the UW OTD donor record and will ship a copy
with the sample (if applicable).
