Title: Ordering Schedule II Controlled
Substances From a Manufacturer or
Wholesaler.

I. PURPOSE: To outline ordering requirements for schedule II controlled substances.

II. POLICY: Schedule II controlled substance inventories are reviewed periodically to
determine what medications are needed. Orders must be placed using a Controlled Substance
Ordering System (CSOS) order or a DEA 222 narcotic order form in addition to a purchase
order.

III. PROCEDURE:
A. Ordering Schedule II Controlled Substances.
1. Periodically the designated pharmacy purchasing staff member will review inventory
levels for schedule II controlled substances. Items falling below the minimum level
will be ordered as needed.
a. Non-formulary schedule IV-V controlled substances will be ordered for inpatients
only if approved by a pharmacy manager.
b. Non-formulary schedule II-III may not be ordered for inpatients.
2. Power of attorney (POA) for the ordering of controlled substances is limited to
managers and designated ambulatory pharmacists. A detailed listing of these
individuals is located in a spreadsheet in the folder J:\Rx\Admindoc\CSOS titled
“CSOS Organization”.
a. A folder for each individual with POA is maintained with signed POA letters in
the office of the Pharmacy Manager of Business Operations.
3. Narcotic order forms (DEA #222) for UWHC can be signed by individuals with POA.
a. When possible the Controlled Substance Ordering System (CSOS) will be used to
place schedule II controlled substance orders.
b. Individuals with POA and who have completed an application for a Controlled
Substance Ordering Certificate can sign electronically CSOS orders for schedule
II narcotics.
4. After a DEA order form 222 is completed the purchasing individual will complete the
following:
a. Send the top two copies of the DEA 222 form along with a copy of the purchase
order to the appropriate vendor. The carbon in between the two DEA forms must
be left in place.
b. Deliver the blue or third copy to the NarcStation or keep on file in the retail
pharmacy (see pharmacy P&P 7.14 Schedule II Controlled Substance Procedures
for Ambulatory Pharmacies for details on record keeping).
5. Narcotic order forms (DEA 222) that have alterations, erasures or strikeovers are
unacceptable and must be redone.

6. If a CSOS order is completed instead of a form #222, the purchasing individual will
print out a copy of the purchase order from the vendor’s system and deliver to the
NarcStation or keep on file in the retail pharmacy instead of the blue or third copy of
form #222.
