38.2


I. PURPOSE

To define a process by which the ED nurse witnesses the wasting of
controlled substances by EMS providers in the ED.


II. POLICY

A. EMS providers may request an ED nurse to witness the waste of
controlled substances that were not used during the transport of a patient
to the ED at University of Wisconsin Hospital and Clinics (UWHC) and
American Family Children’s Hospital (AFCH).
B. The ED nurse will sign the EMS document stating he or she witnessed the
waste of the described medication.


III. PROCEDURE

A. The EMS provider, after giving verbal report to the ED care team, may
request that the ED RN witness the waste of remaining medications.
B. The ED nurse will witness the waste of the medication into the Cactus
waste system, sink or toilet.
1. The ED nurse is verifying the volume of the medication
documented versus being wasted.
2. The ED nurse is verifying what he or she was told is in the
syringe versus the labeling on the medication.
a. The ED nurse is not verifying what the medication actually
is.
C. The ED nurse will sign the EMS record as the witness of the waste.
D. If there are concerns related to the accuracy of the information shared
regarding the medication, the ED nurse will notify the Director of
Emergency Services.
1. Provide the following information.
a. Service
b. Provider name
c. Medication reportedly wasted
d. Patient name
e. Date and time of arrival to the ED
E. The Director of Emergency Services will contact the Chief and the
Medical Director of the involved Service.
