To define a process for billing the initial workers compensation visit for a UWMF employee.
1. UWMF’s Human Resources (HR) department reviews all initial workers compensation
claims for UWMF employees. If the claim falls under a certain dollar limit, the claim is
written off as an employee health adjustment. If it is over a certain dollar limit, it will be
billed to UWMF’s worker’s compensation carrier. Subsequent worker’s compensation
claims are billed to UWMF’s carrier. This policy has been developed to prevent initial
services from being billed to UWMF’s worker’s compensation carrier prior to HR review.
2. Encounter forms should be submitted for all services provided to UWMF employees that are
worker’s compensation related and the visit should be documented in the employee’s medical
