Administrative Departmental Policy
This department-specific policy applies to the operations and staff of the Information Services
Department of the University of Wisconsin Hospitals and Clinics Authority as integrated effective July 1,
Policy Title: IS Policy and Procedure Development

This policy is established to outline the requirements for the creation and maintenance of the
Information Services policy and procedure documents.
II. DEFINITIONS (optional)
OCIO: Office of the Chief Information Office includes the Vice President, Chief Information
Officer, Chief Technology Officer and the Chief Application Officer.
III. POLICY ELEMENTS
A. Policy or procedure proposals should be brought to the IS Director for that particular
area. They will gain endorsement for creation.
B. Policy or procedure title must be approved by the IS Directors and the OCIO,
C. All policies will be reviewed and approved by the IS Directors and the OCIO.
D. Approved documents will be published to the Policies > Administrative > Information
Services page on UConnect.
E. Published policies and procedures will be reviewed on an annual basis by the IS Directors
and the OCIO, and revised every three years, or earlier if there are fundamental changes.
F. IS policy and procedures versions will be kept on file for seven years for auditing
