Policy Title: Patient Safety Escalation

This policy establishes guidelines and procedures for Health Link teams responding to Patient
Safety Escalations and Potential Patient Safety Issue notices from Epic Systems.
Patient Safety Escalation (PSE) and Potential Patient Safety Issue Notices: Notification from
Epic Systems of a patient safety issue related to a particular workflow or design/functionality
issue of the software. Included is a description of the issue, how it can occur, related workarounds
or potential fixes, and a search of affected records.
Epic Systems: Vendor of the Health Link software.
Sherlock: Epic’s ticketing system used to record issues and resolutions for customers.
III. POLICY ELEMENTS
UW Health Link teams respond to Patient Safety Escalations from Epic Systems in a timely
manner. Issues are reviewed by application team leads within one business day of receiving the
notice from Epic. Analysis of the issue may result in workflow or build changes that are
documented and communicated to the appropriate parties.
