CLINICAL POLICY AND PROCEDURE
TITLE: CPR CARD VERIFICTION and CPR CARD REPLACEMENT

PURPOSE: To assist current and former UWMF employees needing verification of their
CPR status, and to assist people in replacing a CPR card.
POLICY: Using the following procedures an employee can use the “Employee Self
Service” to verify CPR status or to request a replacement CPR card.
CPR Card Verification
1. From the main page of Uconnect
a. Click on the Departments tab
b. Click on Human Resources under “UWMF Departments”
c. Scroll down page to Resources box on right hand column
d. Click on “Employee Self Services” ESS
2. In the (ORACLE) box – type in your log-in and password
a. Click on “Employee Self Services”
b. Click on “Training and Development”
c. Click on “Licenses & Certificates”
3. Print out the listing of your CPR status.
CPR Card Replacement
1. Contact the clinic manager. A CPR record with a Xerox copy of each CPR
card is kept with each manager.
If the manager/clinic does not have a current copy of the CPR card.
2. Contact the Clinical Staff Education CPR Coordinator (608-826-6737) to
request a CPR card replacement.
3. Prior to receiving your replacement, please complete the following:
a. Mail a check for $20.00 (made out to UWMF CPR Program) to the
Staff Education Dept – Loc. 414, Administration Office Building
(AOB) 7974 UW Health Ct. PO Box 620993 Middleton, WI 53562
b. Indicate where the CPR replacement card is to be sent.
c. If the CPR card is to be sent to a home address, include a self
addressed stamped envelop.
d. Allow 4-weeks for replacement card.
