PURPOSE

The purpose of this policy is to establish guidelines that include all staff of the University of Wisconsin
Organ and Tissue Donation (UW OTD) department when nominating an individual or family for a third party
award. UW OTD desires to achieve the greatest strategic impact with awards, including UW OTD leadership
team involvement in the nomination process.

DEFINITIONS

Third party awards: Award nominations solicited by partner organizations, e.g. the United Network for
Organ Sharing (UNOS) “National Donor Memorial Award of Excellence,” “Reg Green Award for Organ
Procurement Organization (OPO) Excellence,” or “Lions Eye Bank of Wisconsin (LEBW) Crystal Vision”
awards.

Awards List Worksheet

POLICY

A. UW OTD will be notified about an award by a third party organization.
B. A UW OTD community outreach (CO) team member will denote the award nomination criteria in
the Awards List worksheet.
C. CO team member will review the Awards List worksheet for previously considered candidates.
D. UW OTD staff member will collaborate with the CO team to brainstorm potential award
nomination candidates.
E. The nomination will be presented for consideration to the UW OTD leadership team.
F. The UW OTD leadership team will select a group to draft the nomination and will approve the
final version of the nomination.
G. .
H. A CO team member will denote the nomination information in the Awards List worksheet.
I. The nomination will be submitted to the third party organization.
