PURPOSE
The purpose of this policy is to outline the University of Wisconsin Organ and Tissue Donation (UW OTD)
death record reconciliation process for hospitals in the UW OTD donation service area (DSA).

POLICY

A. UW OTD will assist hospitals in completing a death record reconciliation to verify that all deaths
that occurred in the hospitals were reported, as required per hospital regulatory standards.
B. A summary of all deaths reported to the UW OTD referral service, Statline, will be sent at
minimum monthly to “A”, “B1”, and “C1” hospitals and at minimum quarterly to “B”, “C”, and
“D” hospitals in the DSA (hospital classifications defined in UW OTD policy 4.01).
C. DSA hospital staff will report any discrepancies to UW OTD.
D. Data regarding death record reconciliation accuracy will be provided to each DSA hospital on
either a monthly, quarterly, or annual dashboard depending on the hospital classification as
defined in UW OTD policy 4.01.
