PURPOSE

The purpose of this policy is to define the UW Organ and Tissue Donation (UW OTD) procedures for staff
training and maintenance of training records.

UW OTD Employee Orientation Checklist

POLICY

New Staff Orientation
A. All new University of Wisconsin (UW) Health staff will complete the new staff orientation
provided by UW Health per UW Health policy 9.60. Staff hired into UW OTD that are already
employed by a UW Health entity may not be required to complete UW Health new staff
orientation but will complete the rest of UW OTD staff orientation process.
B. Each UW OTD manager and/or director will review and update the UW OTD job-specific
competencies prior to the start date of his/her new staff.
C. Each manager and/or director will develop a UW OTD orientation plan for his/her new staff
which may include using and/or updating the UW OTD Employee Orientation Checklist.
D. All new staff will complete UW OTD orientation within the first six months of hire. Exceptions
will be approved by the staff member’s manager and/or director.
E. Each manager and/or director will determine how the UW OTD orientation activities will be
documented in the staff member’s training record.
F. A new UW OTD employee may be assigned one or more preceptors at the discretion of the
manager/director.
G. UW OTD managers will retain a copy of the signed Position Description, Core Competency
Assessment, and completed Orientation Checklist for all staff members.


Competency Assessment
H. Competency assessments will be completed for new staff at the end of UW OTD orientation as
detailed in UW Health policy 9.24.

I. Additional competency assessments may be completed at the discretion of each manager
and/or director.
J. As needed, the manager and/or director will work with the employee and coordinate with UW
Health Human Resources to determine if a performance improvement plan is required and will
facilitate the plan.
K. The manager and/or director will retain a copy of the competency assessment (and any needed
follow up documents). The manager and/or director will retain a copy of the competency
assessment (and any needed follow up documents). These should be digital versions that can be
accessed if needed for survey or audit.

Annual and Probational Performance Appraisals
L. UW OTD staff will receive a probationary performance appraisal per UW Health policy 9.26 at
the end of the six month probation period as required per UW Health policy 9.52.
1. The outcome of the probationary performance appraisal may be successful completion,
termination, or an extension or the probation period.
2. These reviews are kept electronically by UW Health Human Resources and are accessible
to the employee via UConnect.
M. UW OTD staff and his/her manager and/or director will complete an annual performance
appraisal thereafter as required per UW Health policy 9.26.

Continual Education
N. Continual education activities may include but are not limited to policy and regulatory updates,
designated requestor training completed per UW OTD policy 1.11, conferences and webinars,
UW Health Learning and Development classes, grand round presentations, departmental
training sessions, reading meeting minutes, releases or updates to standard forms, updated
workflow reference documents, etc.
O. Trainings required for all staff will be assigned by UW OTD managers and directors and their
designees. These will be documented in staff training record In Donor Link.
P. Individual staff members will add additional activities to his/her training record as they are
completed.
Q. Each department has the discretion to track individual and department-specific training in a
manner appropriate to the specific department.

R. UW Health will maintain training records as outlined in UWHC policy 9.19, Internal Education
and Training, and will maintain all performance appraisal records.
S. UW OTD managers may maintain some training records per his/her discretion.
T. UW OTD staff will maintain their own training records in DonorLink.
U. Traveler services will not use the training record in DonorLink. Activities will be documented
using a different method as determined appropriate by the manager and/or director.
