I. PURPOSE

The purpose of this policy is to establish standard reporting of graft function of transplant
recipients.

II. POLICY

It is the policy of the UW Transplant program to adhere to the Organ Procurement and
Transplantation Network (OPTN)/ United Network for Organ Sharing (UNOS), and the Centers for
Medicare & Medicaid Services (CMS) policies data submission requirements, which includes organ-
specific Transplant Recipient Follow-up forms.

III. FORMS

Transplant Recipient Follow-up forms. See Database Office for copy of forms.

IV. PROCEDURE

A. The Transplant Data Office is responsible for completing the UNOS transplant recipient and living
donor follow-up forms accurately and timely.
B. If a patient transfers care to another transplant center, the transplant coordinator will notify the
data office of the patient name and the center. The data office will notify UNOS after confirming
the center has accepted the patient.
C. Patients for whom we have not received lab work results and have not had a visit with a UWHC
Transplant provider within the last 18 months, despite repeated efforts of transplant
coordinators/staff to receive results and schedule appointments, will be deemed lost to follow-
up. The transplant coordinator will notify the data office of such patients. The data office will notify
UNOS.
