I. PURPOSE
The purpose of this policy is to provide standards of care for a multidisciplinary team to ensure that
post-transplant patients are being managed with a consistent, high level standard of care.

II. POLICY
It is the policy of the UW Transplant program to adhere to the University of Wisconsin Hospital and
Clinics (UWHC) standards of patient care, as well as the standards determined by regulatory and
accrediting agencies, such as The Joint Commission (TJC), the Organ Procurement and
Transplantation Network (OPTN)/ United Network for Organ Sharing (UNOS), and the Centers for
Medicare and Medicaid Services (CMS) as well as State and Federal laws.

Patient Guidelines associated with this policy can be found on the hospital intranet

IV. PROCEDURE
A. Every patient is assigned a primary post-transplant coordinator.
B. Under the supervision of physicians, the post-transplant coordinators manage patients after
transplantation. Responsibilities include, but are not limited, to:
1. Outpatient laboratory: authorizing, monitoring, and triaging results based upon
guidelines and protocols.
2. Medication management
3. Arranging and triaging procedures
4. Health maintenance recommendations
5. Triage of medical conditions and symptoms (transplant related and disease specific)
6. Coordination of outpatient Transplant Clinic follow up
7. Requesting psychosocial, dietary, and other consults as clinically indicated
