I. PURPOSE

The purpose of this policy is to establish guidelines for a uniform and systematic method for
managing the transplant patient national waitlist with the United Network for Organ Sharing
(UNOS) and to ensure that all regulatory requirements are met in the process.

II. POLICY

It is the policy of the UW Transplant program to adhere to the University of Wisconsin Hospital and
Clinics (UWHC) standards of patient care, as well as the standards determined by regulatory and
accrediting agencies, such as The Joint Commission, the Organ Procurement and Transplantation
Network (“OPTN”) and the United Network of Organ Sharing (“UNOS”) and the Centers for
Medicare and Medicaid Services (“CMS”), and any State and Federal laws.

IV. PROCEDURE

A. Patient medical, social, and financial information will be updated by a member of the
multidisciplinary team as necessary.
B. If at any time a patient fails to meet selection criteria (see organ specific 'Patient Selection Criteria'
policy) the patient will be removed from the waiting list and notification will be provided, per organ
specific 'Waitlist Removal' policy.
C. The transplant coordinator may place a patient on temporary inactive status for short-term contra-
indications to transplant (i.e. infection) or patient choice and re-activate the patient once the
condition is resolved.
D. Patients will be notified of any significant changes to their status on the waiting list.
E. Updated testing and consultations will be performed according to organ specific waitlist
management guidelines.
