TITLE
UH INTRAOPERATIVE STAFFING PLAN - INPATIENT



I. PURPOSE

To assure quality nursing care and a safe patient environment in the operating room.

II. POLICY

A. The Operating Room will be staffed at all times with a team composed of a Registered Nurse in
the circulating role and a Registered Nurse or a Surgical Technician in the scrub role.
B. A qualified Registered Nurse will be assigned to circulate in each Operating Room.
1. The Registered Nurse (using the nursing process) plans, supervises, and evaluates the
nursing care of each patient.
2. A Registered Nurse or a Surgical Technician is assigned to scrub in each operating room.
C. Surgical Technicians may assist in circulating duties in the Operating Room under the direct
supervision of a Registered Nurse.
1. A Registered Nurse may delegate tasks to the Surgical Technician. The functions of
assessment, documentation, and evaluation may not be delegated.
D. The Nursing Supervisor, charge nurse, or designee of each specialty will assign staff for the following
day’s cases.
1. Assignments are based on staff competence and patient care requirements.
E. During the hours of 0700 - 1700 (Monday, Tuesday, Thursday, Friday) and 0800 - 1700
(Wednesday) there will be staff available to operate a minimum of 25 rooms. From 1700 -
0700 weeknights, and 24 hours on weekends and holidays there will be a triage team available
in-house to start emergencies until the appropriate specialty team arrives.

The number of OR rooms running after 1700 will be determined by the collaboration of the
Medical Director and the OR Director, or other designees, assessing the availability of
anesthesia and nursing resources.

There will be RN’s and Surgical Technicians assigned to work a relief shift (0900 - 1730,
0930 - 1800, 1000 - 1830) to provide lunch and break reliefs as well as provide coverage after
1500.

The Operating Room will be prepared to staff all OR’s at 1500 either by staffing late shifts,
providing call coverage or soliciting volunteers. Overtime will be mandated only when
necessary on the basis of reverse seniority.

The number of OR rooms running will be determined by the Medical Director based on the
availability of anesthesia and nursing resources and locations.

F. The nursing supervisor or designee will be assigned to triage and to facilitate the daily
operative schedule 6:30 AM - 3:00 PM, and 3:00 - 11:00 PM. The 11:00 PM - 7:00 AM
Charge Nurse will triage and facilitate emergencies and preparations for the next day.
G. All Day/PM rotation RN’s and Surgical Technicians will be assigned to one of eight groups
which will rotate to PM’s (1300 - 2130, 1430 - 2300 or 1500 - 2330), for one week every eight
weeks.
H. All Day/NOC rotation RN’s and Surgical Technicians will be assigned to rotate to cover nights
to be determined by availability of permanent night staff with the goal of a minimum of two
RN’s (one charge) and two Surgical Technicians.
I. There will be four RN and four Surgical Technician weekenders which work 0700 - 1900 as
well as two RN and two Surgical Technician weekenders which work 1900 - 0700 every
Saturday and Sunday. One RN per shift is designated as the Charge/Triage nurse and one
Technician will be designated as the Triage person per shift. This fulfills the requirement for
our Level 1 Trauma Center status. These individuals are utilized to begin traumas as well as
provide relief for others working. The other weekender teams cover any surgical procedures. In
addition to the weekenders, each specialty team will provide 24-hour weekend coverage either
with individuals working designated shifts or with call.
1. Call response time of 45 minutes (45 minutes from phone call, dressed in scrubs, and
reporting to charge nurse).
J. A supervisor is assigned for administrative coverage on nights, weekends, and holidays.
