Title:
Convenience Cards


I. PURPOSE

To provide a benefit to staff who volunteer to work at least 3.5 hours following and contiguous to
their scheduled shift.

II. POLICY

Convenience cards will be given to employees by the Care Team Leader (CTL)/Senior Team
Member (STM), or Manager Assistant (at TAC only) when staff qualifies for that benefit.
Employees are qualified to receive a convenience card when they volunteer to stay at least 3.5
hours after their scheduled shift.

III. PROCEDURE
A. Convenience cards are issued by the CTL/STM (Manager Assistant at The American
Center) to qualified staff
B. Convenience cards will be stored on each unit in a secure location.
C. Criteria for issuance of a convenience card:
1. Eligible staff members are Registered Nurses, Nursing Assistants, Health Unit
Coordinators, direct care Technicians, Patient Safety Observers, Medical
Assistants and other direct care providers within nursing.
2. When an eligible employee volunteers to work at least 3.5 hours following and
contiguous to their scheduled shift, a convenience card can be issued.
D. The amount of the card is $6.00.
E. The CTL/STM (Manager Assistant at TAC) will complete the convenience card log with
each occurrence and reconcile at the end of each month. (See directions on Convenience
Card Log at the bottom of the Inventory Audit instructions.)
F. The Nurse Manager is accountable for over sight and adherence to the convenience card
policy and procedure.
1. The manager will review the log at least once per week to assure appropriateness
of convenience card distribution.
2. The manager will reconcile the unit-specific log data to the central monthly report
showing number of cards issued, and will update the unit-specific log to match the
central report data, if necessary.
G. Purchase a UW Health Convenience Card by submitting a Service Now request.

1. Convenience cards can be ordered by a manager, manager assistant, or other
delegate responsible for purchasing items for the department.
2. Include your department’s cost center number with each order.
H. Convenience cards can be used at the following food venues:
1. University Hospital: Four Lakes Café, Atrium Café, Mendota Market, Coffee
Corner
2. American Family Children’s Hospital: Farmer’s Market Café
3. West Clinic: Coffee Corner
4. The American Center: Bistro
