This policy specifies how computer system backups are performed by UW Health Information
Services to ensure mission critical applications and data are adequately preserved and protected
against data loss or destruction.
Data can be destroyed by system malfunctions caused by environmental, accidental, or intentional
means. Adequate backups are critical to the preservation and protection of data and allow data to
be recovered as necessary. In order to minimize any potential loss or corruption of data, the staff
responsible for providing and operating applications/databases must ensure data is adequately
backed up by following established system backup procedures.
This policy applies to system backups performed by UW Health Information Services, including,
but not limited to, the following:
• Health Link Production Database (Cache)
• Clarity (Oracle)
• PeopleSoft (Oracle)
• Microsoft Windows
• Novell Netware
Backup Plan: A procedure for copying application software and data files to an offsite location
other than the originating system.
Backup Types: There are several different types of backup, as defined in the following table.
Backup Type Description
Full Creates a copy of every file on a storage device.
Creates a copy of files that have been changed (modified, revised, or
created) since the last full or incremental backup was performed. This
method may include transaction, journal, or log files. This method is
usually used in conjunction with a full backup.
Maintains an identical copy of data on disk at another site, usually using
hardware or SAN technology in a near, real-time basis.
An alternative method of creating full backups by synthesizing the data
from a previous full backup (from either a regular full or previous
synthetic full backup) and the periodic incremental backups.
IV. POLICY ELEMENTS
All system backups performed by UW Health IS follow the backup plan designated for the
corresponding application/operating system/database unless a system-specific backup plan is
required (as directed by the work unit, a regulatory requirement, or senior UW Health executive
leadership). All plans are reviewed annually by each specified support team/group.
The following information is required for all backup plans:
• Responsible support team/group
• Backup software used (including version)
• Backup storage media
• Backup schedule (including timeframe and backup type)
• Media retention schedule
• Media storage location
