PE.10.10.09 Performance Evaluation & Work Planning
I PURPOSE

To establish the method and time frame for annual Performance Evaluations (not to be
confused with probationary evaluations), conducted by the Facilities & Engineering
Services leadership team.

II POLICY

All employees in the Facilities & Engineering Services Department will have annual
Performance Evaluations. Performance Evaluations will:

a. be based upon the employee's position and the tasks listed in the
employee's position description;
b. include a review of the employee's position description for accuracy and
completeness;
c. include recognition of the employee's strengths and achievements;
d. include a plan for any areas of improvement that may exist.


III FORMS USED Online Employee Performance Evaluation Form

IV PROCEDURE

1. UWHC Human Resources Department (HR) maintains a list of all Facilities &
Engineering Services employees. Each employee will get an annual
Performance Evaluation to be completed no later than June 30
th
.

2. Supervisors will be responsible for informing their employees of the pending
evaluation so that the affected employees may be prepared. Employees will
complete online Self Evaluations before their Performance Evaluation and within
the timeframe identified by their supervisor.

3. Employees will also complete any assigned online Peer Evaluations before their
Performance Evaluation and within the timeframe identified by their supervisor.
Facilities & Engineering Services Trades Lead Workers will complete Peer
Evaluations for their trade’s staff. Appropriate department contacts will complete
Peer Evaluations for Facilities & Engineering Services Zone Mechanics. Facilities
& Engineering Services Supervisors will identify individuals to complete Peer

Evaluations for other Facilities & Engineering Services staff as appropriate. Self
Evaluations and Peer Evaluations will be used as resources for supervisors when
completing Performance Evaluations.

4. During the evaluation meeting, the supervisor and employee will discuss the
tasks performed by the employee. The supervisor and employee will both ensure
that the employee's position description accurately expresses all tasks,
responsibilities and time percentages performed by the employee.

5. Specific training goals will be created to address any areas of improvement and
will be included in the Performance Evaluation review.

6. The online annual Performance Evaluation form needs to be acknowledged by
both the supervisor and the employee.

7. Two weeks before Merit Increases must be submitted, supervisors will have
completed all Performance Evaluations with their staff.

8. Merit Increases for non-represented staff will be determined based on employee
Performance Evaluations and in accordance with HR’s Merit Guideline and
Process.
