PE.07.10.02 Utility Systems Failure Reporting

I. Purpose: To provide utility system failure reporting guidelines that ensures
resolution and correction of utility system failures in the University of
Wisconsin Hospital and Clinics.

II. Policy: All failures of utility systems will be documented by the Facilities &
Engineering Services department. The documentation will include causes
and recommended corrective action. Included systems and associated
managers and supervisors are as listed herein.

IV. Procedure: A. The Control Room Operator marks the “Affects Utility” field on the
Work Request in the Maintenance Management system for any
event that affects a utility system.
B. A Utility System Failure Report Form is filled out by the Control
Room Operator immediately after service is restored.
C. A copy of the form is attached to the associated Work Order in the
Maintenance Management system by Customer Service.
D. The Utility Team reviews Report C_Rpt042A for Work Orders with
the “Affects Utility” field marked that have been generated since the
previous Utility Team meeting.
E. If the Utility Team determines that the event was not a utility event,
the associated Work Order is marked as not a utility event and as
reviewed by the Utility Team.
E. If the Utility Team determines that the event is a utility event, the
associated Work Order is marked as a utility event and as reviewed
by the Utility Team.
F. If the root cause is unknown, a member of the Utility Team will be
assigned to further investigate and recommend corrective action to
prevent recurrences.
G. If the root cause is known, a Performance Improvement Initiative
will be started and reported to the Environment of Care Committee.
H. Documentation is attached to complete Work Order with notes.
