Title:
Quarterly Check-ins

I. PURPOSE

To ensure employees consistent, continuous and regular feedback, quarterly check-ins will be
completed three times per year to discuss the business goals set in the performance review and
help employees carry these goals to completion.

II. POLICY

All staff will receive check-ins three times per year in addition to their annual performance
evaluation after they have successfully completed their six month probationary evaluation. This
policy is supported by the UWHC Policy 9.26 Probation and Annual Performance Appraisals.

III. PERSONS AFFECTED

Culinary Services staff, including all supervisors and managers.

VI. PROCEDURES

A. Employee Work Performance Evaluation
1. Employees will receive an annual evaluation per Policy 9.26 using the UWHC
Performance Appraisal System in addition to quarterly check-ins three times per
year per the following schedule:
a. 1st Quarter: no later than 9/30
b. 2nd Quarter: no later than 12/14
c. 3rd Quarter: no later than 3/21
2. Supervisors and managers will meet with each employee to review work
performance a minimum of 1 time each quarter.
3. If an employee receives a “2” or less in any area during their quarterly check-in or
during their annual performance evaluation, the overall score for the evaluation will
also reflect a “2” or less and the employee will be placed on a performance
improvement plan.
4. Probationary, quarterly and annual evaluations will only be completed by
supervisors and managers who are not in a probationary status of employment.
