Title:
Approval Process and Working
Relationship between Preceptors and
Nutrition Students


I. PURPOSE:

The purpose of this document is to describe the policies and procedures that guide the
approval process and working relationship for nutrition students obtaining clinical and other
learning experiences at UW Hospital and Clinics (UWHC).

II. POLICY:
A. UWHC clinical nutritionists have a responsibility to participate in the education of
nutrition students.
B. UWHC clinical and administrative policies and procedures will be followed by clinical
nutrition faculty and students.
C. Agreements are developed between UWHC Clinical Nutrition and UW-Madison, or other
schools, for the purpose of providing a variety of learning opportunities including clinical
and classroom for students. No educational experience can take place without a current
contract. If the student is from a school not regularly affiliated with UWHC, discussion
should occur with the Director of Clinical Nutrition and/or the Director of the Dietetic
Internship Program for contracting before plans are agreed upon.
D. UWHC Clinical Nutrition makes decisions about granting clinical placements based upon
a number of factors including but not limited to pre-existing collaborative relationships
(such as with UW-Madison), ability to meet demand, and organizational priorities.
E. The School is responsible for completion and record-keeping of each student's
background check, health screening, Health Insurance Portability and Accountability Act
(HIPAA) review and other required training.
F. A UWHC registered dietitian maintains responsibility for all patients assigned to a
student.
G. The Director of Culinary and Clinical Nutrition, the Clinical Nutrition Manager, and/or
the Program Coordinator of the Dietetic Internship Program, along with the clinical
nutrition preceptors, mutually agree upon the student assignment.
III. PROCEDURES:

A. Appropriate administrative personnel from a education program negotiate with the
Director of Clinical Nutrition and/or the Director of the Dietetic Internship Program for
the selection of sites to be used for clinical learning.
1. The Director of Culinary and Clinical Nutrition, the Clinical Nutrition Manager,
and/or the Program Coordinator of the Dietetic Internship Program will confirm
or deny requests after consultation with the faculty.
2. Decisions will be communicated in writing to the originating representative and
to the Director involved.
3. One-time short observational opportunities in other areas by students already
practicing here can be arranged informally by the faculty member and the
Director of Culinary and Clinical Nutrition, the Clinical Nutrition Manager, and/or
the Program Coordinator of the Dietetic Internship Program.
B. Responsibilities of Clinical Nutrition:
1. The Program Manager of the Dietetic Internship Program, will:
a. Alert preceptors to new procedures, policies, student changes or
concerns which might affect students’ learning opportunities.
b. Ensure that a UWHC clinical nutritionist is assigned to the students and
that the clinical nutritionist is aware of their role with the patients and
students.
c. Institute changes in student assignments as needed.
2. The Clinical Nutrition staff will:
a. Maintain primary responsibility for patient care.
b. Alert the Director of the Dietetic Internship Program to learning
opportunities that may occur on a day-to-day basis or in the long-term.
c. Participate in orienting students as needed.
d. Report to the student and faculty member pertinent information about a
patient that is not available in the chart.
e. Co-sign any narrative notes or write a clarifying note, if the assessment,
intervention, and/or response differs from that which is documented. In
the absence of a narrative note, the clinical nutritionist caring for the
patient will write
f. Interact with the student in a manner appropriate to the student's
learning needs.
g. Report to the Program Manager of the Dietetic Internship Program any
unusual incidents occurring during the student's clinical experience.
3. The student is responsible and accountable for his/her behavior within UWHC.
Therefore, she/he will:
a. Conform to dress and behavior codes governing the actions of
professional nurses at UWHC.
b. Follow UWHC policies and procedures.
c. Perform and complete all activities assigned by clinical faculty.
d. Report to the clinical nutrition staff responsible for the patient and to
clinical faculty:

i. Inability to perform any patient care as soon as the situation is
perceived.
ii. Any changes or incidents concerning the patient.
iii. Unscheduled medication and treatments administered.
e. Provide hand-off report to the staff nurse responsible for the patient
before leaving the unit.
C. Problem resolution:
1. A problem arising between a student, clinical nutrition staff member, or a
preceptor will be discussed with involved persons at the time of occurrence.
2. If a problem is not resolved, assistance in problem solving will be sought from
the Director of Culinary and Clinical Nutrition, the Clinical Nutrition Manager,
and/or the Program Coordinator of the Dietetic Internship Program.
D. Graduate clinical nutrition students will work with a designated faculty member to
negotiate for independent learning opportunities with the Director of Culinary and
Clinical Nutrition, the Clinical Nutrition Manager, and/or the Program Coordinator of the
Dietetic Internship Program Director of Clinical Nutrition and/or the Director of the
Dietetic Internship Program.
1. After agreeing to serve as a preceptor for an independent experience, the
Director of Culinary and Clinical Nutrition, the Clinical Nutrition Manager, and/or
the Program Coordinator of the Dietetic Internship Program will work with the
graduate student and faculty member to come to agreement on the student
objectives, time, and date for the experience.
2. The Director of Culinary and Clinical Nutrition, the Clinical Nutrition Manager,
and/or the Program Coordinator of the Dietetic Internship Program will inform
the appropriate individuals of the student's appointment and independent
learning plan.
