Whether you’re closing on buying a house or signing a lease at a new apartment, moving involves lots of little details that might get overlooked amid the flurry of cardboard boxes and packing tape. Below is a guideline and a printable checklist to help you remember a few of the essentials, both within UW Health and outside the organization.

Within UW Health
To update your personal information, such as address and phone number, please visit the Personal Information page of Employee Self Service to update with Human Resources.
Once your address is updated in Employee Self Service, Human Resources receives a weekly report to update your address with your benefit vendors. Other than updating your address with Employee Self Service, there is no additional action you need to take to update it with your benefits.
Outside UW Health
- Notify the post office well in advance of your move so that you may begin receiving mail at your new address. The U.S. Postal Service offers a step-by-step online Mover’s Guide to help you make the proper notifications.
- Update your driver’s license. According to the Wisconsin Department of Transportation, you should notify the DOT of your change of address within 10 days of your move.
- Notify the IRS and other governmental agencies. Because the IRS sends correspondence to the last known address of the taxpayer, neglecting to notify the IRS of your new information may cause delays or failure to receive important notices – potentially resulting in costly consequences.
- Contact your medical insurance company to update your address and phone number. Also, if you’ve moved to an entirely new area, request physician recommendations near your new home. Also, ask your current physician(s) to forward your medical records to your new provider.
- Contact your bank to change your address if you’re moving locally. If you’re moving to an entirely new area, you may need to close your old account and contact local banks to establish new accounts. Also, contact your credit card companies, student loan creditors and other creditors.
- Don’t forget to close or transfer your utilities, cable and Internet service. If you subscribe to magazines or newspapers, call their circulation departments to either cancel or transfer your subscriptions.
