Below is a guideline and a printable checklist to help you remember a few of the essentials, both within UW Health and outside the organization.

Within UW Health

To update your personal information, such as address and phone number, please visit the Personal Information page of Employee Self Service to update with Human Resources.

Once your address is updated in Employee Self Service, Human Resources receives a weekly report to update your address with your benefit vendors. Other than updating your address with Employee Self Service, there is no additional action you need to take to update it with your benefits.

Outside UW Health
