Focus

Whether you’re closing on buying a house or signing a lease at a new apartment, moving involves lots of little details that might get overlooked amid the flurry of cardboard boxes and packing tape. Below is a guideline and a printable checklist to help you remember a few of the essentials, both within UW Health and outside the organization.

We hope that you find the resources valuable as you plan for your move. You can find additional information on all benefits and benefit programs by visiting the Benefits page. If you have any questions, please contact Human Resources at (608) 263-6500

Within UW Health

To update your personal information, such as address and phone number, please visit the Personal Information page of Employee Self Service to update with Human Resources.

Once your address is updated in Employee Self Service, Human Resources receives a weekly report to update your address with your benefit vendors. Other than updating your address with Employee Self Service, there is no additional action you need to take to update it with your benefits.

Outside UW Health
