Focus

Important Deadline

You have 30 days from the date of divorce decree to remove a dependent from your insurance, which includes removing a dependent from your insurance. Forms must be completed, submitted to, and received by HR via:

If you do not remove your dependent within the 30-day deadline, you must wait until the next available Open Enrollment period to make changes to your benefits enrollment and the change will be effective January 1.

When submitting new enrollment forms, please be sure to use your current legal name (name on your Social Security Card). HR will update your name with all your benefits once we receive your new Social Security card, as outlined above.

Health Insurance

If you wish to change your current medical election (newly enroll, add/drop dependent(s), or cancel), you will need to submit a health insurance enrollment form.

This form needs to be completed and submitted to HR within 30 days from the date of divorce decree. The coverage is effective the date following the termination of your other coverage (i.e., if the coverage terms May 25, the coverage through MF is effective May 26.) New medical enrollment through UWMF is effective the date following the termination of your other coverage. If removing a dependent from your plan, their UWMF coverage will last through the end of the month in which the divorce occurred.

If you wish to cancel your current medical coverage, you must gain new coverage. If you wish to newly enroll in a UWMF health insurance plan, you must lose other coverage. You are required to submit a letter from your insurance carrier with your health or dental application due to your qualifying event indicating the gain/loss of other coverage, including effective date, group number and members that are gaining/losing coverage.

Dental Insurance

If you wish to change your current dental election (newly enroll, add/drop dependent(s), or cancel), you will need to submit a dental insurance enrollment form.

This form needs to be completed and submitted to HR within 30 days from the date of the divorce decree. New dental enrollment through UWMF is effective the date that your other coverage ends. If removing a dependent from your plan, their UWMF coverage will last through the end of the month in with the divorce occurred.

Supplemental Benefits

There are other benefits an employee can change due to a qualifying life event. Below is a list of benefits an employee could change if eligible for the benefits. Beneficiary Information

If you wish to change your beneficiary for your life insurance(s) and/or retirement plan, you will need to complete the required beneficiary forms.

Retirement

Often when employees go through a divorce, they are required to separate the balance of their retirement accounts through a Qualified Domestic Relations Order (QDRO). If there is a QDRO applicable to your UWMF retirement account, please contact Human Resources directly.
