Important Deadline

You have thirty days from the date of divorce decree to make changes to your benefits. UW Medical Foundation enrollment forms must be completed and submitted to HR via fax: (608) 263-5778 or by attaching to an Ask HR case in ServiceNow.

To make enrollment changes to your benefits through UW School of Medicine and Public Health, contact Employee Services at (608) 262-5650.

If you do not enroll your new dependent within the 30-day deadline, you must wait until the next available Open Enrollment period to make changes to your benefits enrollments and the change will be effective on January 1.

Health & Dental Insurance

UW Medical Foundation – Dental Insurance

If you wish to change your current dental election (newly enroll, add/drop dependent(s), or cancel), you will need to submit a dental insurance enrollment form.

This form needs to be completed and submitted to HR within 30 days of the date of divorce decree. New dental enrollment through UWMF is effective the date that your other coverage ends. If removing a dependent from your plan, their UWMF coverage will last through the end of the month in which the divorce occurred.

UW School of Medicine and Public Health – Health Insurance

Physicians receive health insurance benefits through UW School of Medicine and Public Health. To add your child to your health insurance coverage or your supplemental major medical and dental insurance from Epic, contact Employee Services at (608) 262-5650.

UW School of Medicine and Public Health – Dental Insurance

You may also have basic preventive coverage from Blue Cross/Blue Shield or supplemental major medical and dental insurance from Epic. Both plans are administered by the UW School of Medicine and Public Health. You may call Employee Services at (608) 262-5650 to enroll your child.

Life Insurance

UW Medical Foundation

If you are enrolled in UWMF-administered dependent life insurance, you may need to make changes to your coverage due to your divorce. Dependent Life insurance coverages all eligible dependents, including children and spouse/domestic partner. If you need to enroll or make changes to your Dependent Life insurance as a result of your divorce, you need to submit a completed Dependent Life insurance application to Human Resources.

You may also want to update your beneficiary for your Life Insurance policies through UW Medical Foundation. This can be done at any time. To make the update, this form needs to be completed and returned to Human Resources.

UW School of Medicine and Public Health

To change beneficiaries for your University Insurance Association Life Insurance and any option life insurance policies administered by UWSMPH, contact Employee Services at (608) 262-5650.

Retirement

UW Medical Foundation

To update your beneficiaries for your UW Medical Foundation Physicians Retirement Plan, you need to complete a new Beneficiary Designation Form and return it to Human Resources.

UW School of Medicine and Public Health

To change beneficiaries for your WRS Retirement Plan from the state or your 403(b) tax-sheltered annuity plan with UWSMPH, contact Employee Services at (608) 262-5650.

Flexible Spending

UW Medical Foundation

As a result of the qualifying event, you can make changes or enroll in the UWMF Health Care Flexible Spending program within 30 days of the qualifying event date.

UWMF offers only the health care flexible spending option – there is no dependent care option through UWMF. If you want to enroll in this option, you must do so through UWSMPH.

To make changes or enroll, you will need to complete this form and return it to Human Resources within 30 days of the qualifying event date.

UW School of Medicine and Public Health

As a result of the qualifying event, you can make changes or enroll in the UW’s Employee Reimbursement Account. The ERA offers an account for health care and dependent care expenses. For more information, contact Employee Services at (608) 262-5650.

As a result of your qualifying event, you have the opportunity to make changes or enroll in the LifeLock Identity Theft Protection program with UW Medical Foundation.
