This term life insurance policy is equal to one and a half times your prior calendar year W-2 wages. At time of new hire, this benefit is based on your starting salary. Coverage includes Accidental Death & Dismemberment for the same amount of coverage.
Full time staff who work 30 hours/week or more and APPs who work 20 hours/week are automatically enrolled in this benefit the first of the month following one month of employment.
This benefit is provided to you by the UW Medical Foundation. There are no employee premiums for this benefit.
You can update your beneficiary for this term life insurance policy at anytime. In the State of Wisconsin, your spouse must be listed as your 100% primary beneficiary, unless a spousal consent form is signed and notarized. To update your beneficiary, complete the Life Insurance and AD&D Beneficiary Form and submit it to UW Health Human Resources.
New employees are enrolled in this benefit the first of the month following one month of employment.
If you move into an eligible position for this benefit (full time staff: 30 hours/week or more, APPs: 20 hours/week or more) are automatically enrolled in this benefit.
