Dependent Life Insurance provides life insurance coverage for spouse/domestic partner and eligible dependents.
The plan offers $10,000 of benefit for spouse/domestic partner and $5,000 per child. An eligible dependent under this plan is a dependent child up to age 19 if they are not a full-time student, or up to age 25 if they are a full-time student, unmarried and not on active military duty.
Dependent Life Insurance is a blanket policy, meaning it covers any spouse/domestic partner or eligible dependents at a standard monthly rate, regardless of the number of dependents covered. For this reason, Human Resources does not require the name of the dependents you wish to cover with this plan. As long as you have at least one eligible dependent to cover with this benefit, you can choose to be enrolled.
If and when you no longer have any eligible dependents to cover with this benefit, it is your responsibility to notify Human Resources so we can assist you in canceling your coverage.
Physicians with a 50% appointment or greater are eligible for dependent life insurance.
Dependent Life Insurance is a blanket policy, meaning it covers any spouse/domestic partner or eligible dependents at $0.93 a month, regardless of the number of dependents covered.
There is no beneficiary designation form for this benefit, as you are the beneficiary for your dependents.
Enrollment in Dependent Life Insurance requires submission of the to Human Resources within 30 days of the date of hire. Coverage will be effective the first of the month coinciding with/following one full month of employment.
Qualifying Life Event
Changes may be made to your Dependent Life Insurance enrollment if a qualifying life event occurs. To make changes, please review Life Events (UWMF) page on UConnect for information on what changes you can make due to a Qualifying Life Event, how to obtain applications, and when the change is effective. Completed applications must be submitted to Human Resources within 30 days of the qualifying life event.
A qualifying life event includes marriage, birth or adoption of a child, a death, divorce, termination of employment, or for dependent care only, a change in childcare providers or cost.
An annual enrollment period is offered for Health Insurance each year during the Open Enrollment period in October/November. This is an employee’s opportunity, outside of a qualifying life event, to make changes to your benefits.
