Locating a VSP Network Provider

To locate a VSP network Provider visit the VSP Web site at www.vsp.com/go/stateofwiemployees. When visiting the website, select “Find a VSP Doctor” and enter the desired address or zip code to view VSP’s directory. Provider information is also available by contacting VSP’s Customer Service center at (800) 400-4569.

Benefit Eligibility

All regular and temporary employees who are eligible who become eligible for the Wisconsin Retirement System (WRS) are eligible to participate. 

For temporary employees, coverage with UWHC contribution is effective the 1st of the month following six months of Wisconsin Retirement System (WRS) participation. UWHC will pay 50% of the total monthly premium for the medical plan you elect. Until eligible for the UWHC contribution, temporary employees can elect early coverage and pay 100% of the premium. Early coverage can be effective the 1st of the month following the WRS eligibility date.

Initial Enrollment

To enroll in the VSP Vision Insurance plan, the enrollment form must be submitted to Human Resources within 30 days of the date of hire. Coverage will be effective the first day of the month following receipt of the application.

Open Enrollment

The opportunity to enroll in the VSP Vision Insurance plan is offered each year during Open Enrollment. Open Enrollment for this plan typically occurs in the fall for a coverage effective date of January 1 of the next year.

Qualifying Life Event

Changes may be made to your VSP Vision Care insurance if a qualifying life event occurs. To make changes, please review Life Events (UWHC) page for information on what changes you can make due to a Qualifying Life Event, how to obtain the applications, and when the change is effective. Completed applications must be submitted to UW Health Human Resources within 30 days of the qualifying life event.

A qualifying life event includes marriage, birth or adoption of a child, a death, divorce, termination of employment, or for dependent care only, a change in childcare providers or cost.


Canceling Coverage

Cancellation of VSP Vision Insurance plan benefits is permitted; however, enrollment in the coverage must continue through the full calendar year. To cancel the VSP Vision Insurance plan for the upcoming calendar year, please complete the VSP Enrollment Form. Cancellation forms must be submitted to UW Health Human Resources prior to December 1. Coverage and benefits will terminate effective January 1 of the next year.

Open Enrollment provides another opportunity for you to cancel coverage for the following calendar year. Information regarding canceling as part of Open Enrollment will be communicated during October/November.
