Benefit Summary

To encourage regular dental care, UWMF offers a comprehensive and generous dental plan through Delta Dental of Wisconsin. The plan offers 100% coverage on preventative and basic restorative services like routine 6-month cleanings, cavities, etc. UWMF's plan also offers $2,000 lifetime maximum for orthodontia for each family member. Premiums are paid by the physician. 

Benefit Eligibility

To be eligible for UWMF's dental insurance, you must have a 50% appointment or greater.

Physicians are also eligible for UW Medical School dental insurance. Please contact your department administrator for more information.

Single Coverage - $40.24/month

Family Coverage - $114.39/month                              

Enrollment

Initial Enrollment

Enrollment in dental insurance requires submission of the Delta Dental application to Human Resources within 30 days of the date of hire. Coverage will be effective the first of the month coinciding with/following one full month of employment.

Qualifying Life Event

Changes may be  made to your dental insurance if a qualifying life event occurs. To make changes, please review Life Events (UWMF) page on UConnect for information on what changes you can make due to a Qualifying Life Event, how to obtain applications, and when the change is effective. Completed applications must be submitted to Human Resources within 30 days of the qualifying life event.

A qualifying life event includes marriage, birth or adoption of a child, a death, divorce, termination of employment, or for dependent care only, a change in childcare providers or cost.

Open Enrollment

An annual enrollment period is offered for dental insurance each year during the Open Enrollment period in October/November. This is an employee’s opportunity, outside of a qualifying life event, to make changes to your benefits.
