Benefit Summary

In the event of a long-term serious health condition resulting in a loss of income, we offer you long-term disability coverage. After 90 days of disability, you are eligible to receive 60% of your salary or 66-2/3% if you are a mid-level provider, director, or executive. Your premiums are paid by UWMF.

Benefit Eligibility

To be eligible for long term disability, you must work at least 30 hours per week (equivalent to a 0.75 FTE).

Long-Term Disability Coverage

Enrollment

If eligible, you are covered by disability insurance effective the first of the month following one month of employment. You will need to complete the and make an election to enroll in either the pre-tax or post-tax option. You can only change this election during Open Enrollment, provided you are not currently receiving long-term disability benefits.
