In the event of a long-term serious health condition resulting in a loss of income, UW Medical Foundation (UWMF) offers you long-term disability coverage. Your UWMF income is insured by a group disability plan through The Hartford Insurance Company. This group plan provides a benefit equal to 66-2/3% of your UWMF income, to a maximum monthly benefit of $20,000.
The group plan insures you in your speciality and/or subspecialty as it pertains to your practice. The premium for this plan is paid by you. Any benefits you receive under this plan would be income tax-free, under current IRS and Wisconsin regulations. The monthly premium is based on your salary; the current formula is Annual Salary x 1.25/ 100/ 12. For newly hired physicians, the Annual Salary is determined from the appointment letter. For subsequent years, the Annual Salary is determined by the prior year UWMF W2 salary (as long as one year of employment was completed during the tax year).
To be eligible for long term disability, you must have a 50% appointment or greater. Basic Long Term Disability is a mandatory benefit.
How to File a Claim
Contact Gallagher Benefit Services at (608) 274-8989 to initiate a claim for Long Term Disability benefits. Gallagher will require the physician name and employer (University of Wisconsin Medical Foundation) to begin the claim intake process.
