LifeLock Benefit Elite (only available as a payroll deducted employee benefit) includes searching over a trillion data points every day for potential threats to your identity and to financial assets – your 401(k) and investment accounts. Also includes scanning for misuse of your Social Security number, change of address and court records scanning for use of your identity to commit crimes.
LifeLock Ultimate Plus™ service provides some peace of mind knowing you have LifeLock’s most comprehensive identity theft protection available. Enhanced services include bank account application and takeover alerts, online credit reports and credit scores.
LifeLock is a benefit available to UW Medical Foundation employees. To be eligible, you must work at least 15 hours per week (equivalent to a 0.38 FTE).
The premiums are based on the type of coverage, LifeLock Elite or LifeLock Ultimate, as well as the tier; Employee Only, Employee + Spouse/Domestic Partner, Employee + Child(ren), Employee + Family. Premiums are post-tax deductions. Employees save 15% on premiums by enrolling in LifeLock Identity Theft Protection through UWMF.
Enrollment in LifeLock Identity Theft Protection requires submission of the LifeLock Identity Theft Protection application to Human Resources within 30 days of date of hire. Coverage will be effective first of the month coinciding with/following one full month of employment.
Qualifying Life Event
Changes may be made to your LifeLock Identity Theft Protection enrollment if a qualifying event occurs. To make changes, please review the Life Events (UWMF) page on UConnect information on what changes you can make due to a Qualifying Life Event, how to obtain applications, and when the change is effective. Completed applications must be submitted to Human Resources within 30 days of the qualifying life event.
A qualifying life event includes marriage, birth or adoption of a child, a death, divorce, termination of employment, or for dependent care only, a change in childcare providers or cost.
An annual enrollment period is offered for Health Insurance each year during the Open Enrollment period in October/November. This is an employee’s opportunity, outside of a qualifying life event, to make changes to your benefits.
