Any situation or occurrence which is not consistent with routine operations at UWHC may be a safety issue. These issues should be reported in Patient Safety Net at UWHC Patient Safety Net (PSN).

Patient Safety Net (PSN) is an online data collection and reporting tool used to identify and report adverse medical events, potentially harmful situations and ‘near misses’ that could result in harm to patients, staff members and visitors. PSN reports are a critical source of information about systems issues that affect patient safety. The goal is to increase the quantity and quality of occurrence reporting at UWHC.

PSN is intended to facilitate collection of information at the point of care, encourage identification of the root causes of adverse events, and promote system redesign to prevent recurrences. PSN can facilitate benchmarking and collaborative sharing of best practices to reduce errors.

For more immediate or urgent issues, employees may call Safety at (608) 263-1512 or send an email to uwhcsafety@uwhealth.org to report the incident or issue; however, a PSN will still need to be entered for tracking purposes.
