A criminal background check

The background check is completed by the SMPH Dean’s Office human resources and kept on file there. Individual department hr staff will coordinate the checks with the Dean’s Office.

The background check is completed by Waisman Center clinical services administration.

A TB (Tuberculosis) test within the past 12 months (needed only if the employee has direct patient contact)

To obtain a TB test, contact your department’s human resources office for instructions. When your TB test has been completed, that office will provide necessary certification to the SMPH Dean’s Office human resources.

To obtain a TB test, contact the Waisman Center clinical services administration for instructions.

HIPAA training

To accomplish this training, go to the University of Wisconsin HIPAA Web site at http://www.wisc.edu/hipaa/. When you have completed the training, print two copies of your training certificate, keep one copy and give the other copy to your department’s human resources department.

Upon completion of the steps above, UW Health compliance and information technology services will be notified by the SMPH Dean's Office of human resources or Waisman Center clinical services administration that your compliance requirements have been met.
