How to Fulfill the Requirement

Document your research role.

Because of federal action regarding the privacy of patient records (HIPAA), UW Health requests additional documentation from research staff prior to authorizing access to systems containing patient data. This information is necessary so UW Health can document the intended use of electronic Protected Health Information by research staff, and assure such use is consistent with UW Health and UW-Madison regulations. During the audit trail review process, UW Health and SMPH may also use the documentation to help determine whether research staff are using data only for tasks that are consistent with institutional review board (IRB)-approved protocols.

Submit the following information to the department administrator in your SMPH department:

  1. A brief overview of the IRB-approved research project(s) in which you are involved, especially including your typical subject identification or recruitment methodology.
  2. Your role in this research
  3. A specific description of how you will utilize Health Link or other UW Health systems in the context of this research. This description should address the specific Health Link functions you are requesting via the authorization form. For example, if you request access to clinic schedules, please describe how you will use the schedules in the context of your research.
  4. Your clinical credentials (e.g., RN, LPN, etc.), if applicable

Establish baseline authorization from your hiring authority for access to Health Link

The authorization form must be completed accurately and in its entirety. An SMPH department/division administrator must sign at the bottom to authorize access. Authorization signatures of other personnel will not be accepted.

When you have completed all of these steps, a representative from the UW Health information technology services department will send an email containing your Health Link user ID and temporary password to the person who authorized your access to Health Link. You will then be able to log in to the Health Link system, as follows

Create a secure Health Link password

When you receive your Health Link User ID and temporary password, you will need to establish a secure password using the online password change assistant.

Confirm that your password is working properly by using it to log in to Health Link.

To log into Health Link from an SMPH or Waisman network:

  1. Navigate to https://uconnect.wisc.edu.
  2. Click on the Health Link HC link.
  3. Enter your Health Link ID and password.
