Requirement

SMPH Employees - How to Fulfill the Requirement

A criminal background check or a background check in accordance with the Wisconsin Caregiver Background Check Law

The background check is completed by the SMPH Dean's Office human resources and kept on file there. Individual department hr staff will coordinate the checks with the Dean's Office.

A TB (tuberculosis) test within the past 12 months (needed only if the employee has direct patient contact)

To obtain a TB test, contact your department’s Human Resources office for instructions. When your TB test has been completed, that office will provide necessary certification to the SMPH Dean’s Office Human Resources.

HIPAA training

To accomplish this training, go to the University of Wisconsin HIPAA Web site at http://www.wisc.edu/hipaa/. When you have completed the training, print two copies of your training certificate, keep one copy and give the other copy to your department’s Human Resources Department.

Safety and infection control training
(needed only if the employee has direct patient contact)

To complete this training, contact your department’s Human Resources office for instructions. When your training has been completed, that office will provide necessary certification to the SMPH Dean’s Office Human Resources.

Verification of licensure, if required by law, regulation or UW Health policy

You must provide verification of your licensure to your department’s Human Resources office.

Upon completion of the steps above, UW Health compliance and information technology services will be notified by the SMPH Dean's Office of human resources that your compliance requirements have been met.

Special Note for RN Researchers

Researchers who are also licensed RNs must obtain approval from the UW Health Nursing Education Department and RN Research Practice Committee prior to practicing as an RN in the UW Health clinical environment. When this approval is obtained, you will also be granted RN level Health Link security (as applicable) to enable you to practice within the full scope of your RN license within Health Link, such as to sign orders explicitly stipulated by the research study. Please refer to UW Health Policy 9.85 to access the application or the Research RN's page for additional information about this process.
