Learning Center/Patient Relations/CSC Clinics Process - UW Medical Foundation, Patient Resources, Social Work Services Quick Guide, Social Work Manual, Advance Directives, POA-Health Care, POA Outpatient Process

Focus

COC inpatient triage social workers (medicine and general) assist the Learning Center when paged to assist with both explaining and witnessing an AMD.

At times the outpatient clinic triage person has been called to assist. Since the clinic triage person is not always located in the CSC, the practice is that the COC inpatient triage social workers are called first, and they may choose to call a clinic social worker if appropriate. Clinic social workers should not be called directly for witnessing by clinics, the Learning Center, etc.

Assistance should also be provided to Patient Relations for witnessing.
