- A Living Will is a declaration to physicians that you wish life-sustaining procedures to be withheld or withdrawn if you are in an incurable condition and your death is imminent
- If you have both a Living Will and a Power of Attorney for Health Care, the Health Care Power of Attorney controls if there is any conflict between the two
- Copies of patient's Living Wills should be filed in charts, marked with an orange sticker, and sent to scanning
- To print out an Advance Directive that has been scanned into WISCR-IT: bring up patient’s record, and click on “Scanned Documents”. If patient’s document has been scanned in, it will show up. Note that not every Advance Directive we have on file has been scanned in. If you come across a paper copy of a document that has not yet been scanned in, please send to Sue Esch in Medical Records at tube # 841 or fax # 2-6663 (use scan form).
