The website will provide health insurance plan options and will also notify you if you are eligible for a subsidy, cost share reductions, or Medicaid.
NOTE: The Affordable Care Act requires all US citizens to have health insurance. You must enroll during the open enrollment period or qualify for a special enrollment period.
- BadgerCare/Medicaid (MA)
For patients age 65 or older, under 19, blind or disabled, pregnant, parents and relatives caring for a child, parents with children in foster care, farmers, and other self-employed parents. 608-355-4200
- Foot Care Clinics
Foot clinics are staffed by Home Care employees and are held throughout the county in Baraboo, LaValle, Merrimac, Plain, Sauk City, Reedsburg, and Spring Green. Services provided include foot soaks, toe nail trims, nail filing, callous care, lotion, and foot massage. Cost is $25.00 for each appointment. 608-355-4313
