Free senior care referral service specializing in assisted living, nursing homes, retirement communities, personal care homes, Alzheimer’s care,
and home care
Referral assistance, family support, family care consultation, family education. Alzheimer’s Family and Caregiver Support Program (AFCSP): Support and limited stipend to qualified families caring for a person
with Alzheimer’s disease or related dementia.
Area Agency on Aging
Services available for caregivers helping to care for someone age 60 years and over. Sample of services: Case management, caregiver seminars, support, training, Lifeline, financial assistance, legal
assistance, respite care, telephone reassurance, and transportation
CARE Wisconsin — Partnership
Purpose: Help adults live independently at home and in the community. Criteria: Age 18 or over and have a developmental and/or physical disability or age 55 or over; with long-term care needs; meet functional and financial requirements, (Medicaid or Medicaid eligible. CW will determine); and be a resident of Dane County.
Sample of services: Care management by interdisplinary team including nursing care, in-home services; coordination of non-medical issues (housing/transportation), health and medication monitoring, crisis intervention, adult day services, 24-hour phone coverage by RN.
Catholic Charities Aging Services
Companion Care: Sliding scale, transportation and escort, light meal prep, light housekeeping, bath assistance, respite, companionship, and
specialized dementia care.
Elder Mentor: Volunteer mentors, free advice, info, assistance to elders (checkbook balancing, companionship, organizing materials, etc).
Respite Care: Rest and relief for caregivers and
socialization for homebound seniors.
Coalitions for Older Adults
Outreach workers coordinate services for elderly persons such as: Financial programs, nutrition sites, transportation, attendant care, housing, home care, etc.

Coalition of Wisconsin Aging Groups (CWAG)
Statewide assistance, resources and advocacy for seniors. Also operates the Elder Law Center providing investigation, research and public education
on general issues of elder law. Provides direct services on issues of public benefits and health care financing to Dane County seniors age 60 and older.

- Retired Senior Volunteer Program (RSVP)
Trained volunteers can provide a free in home safety assessment and falls prevention, some free safety devices and information on home security, emergency and safety tips. These services are available to adults of all ages.
