Social Service Resource Information
When outpatient social service needs are identified, Patient Resources can assist by providing referral information about financial/insurance issues, transportation, nursing home/assisted living placement, housing/shelter, home health services, mental health, abuse/neglect issues, pregnancy issues, etc.
Medical Assistance patients with appointments at UW Health out-patient clinics should call MTM, Inc. for transportation needs: 1-866-907-1493. For patients who do NOT have Medical Assistance, a Patient Resources Representative can provide information on any other transportation resources available.
Physician Referral and General UW Health Information Line
We provide physician referral information to patients seeking primary care or specialty services and we provide general information about UW Health.
New or displaced patients are able to speak to a Triage RN who assesses health needs, updates HealthLink with the needed health history (i.e., medical/family history, medications, allergy information), schedule mammograms as appropriate and arranges an encounter with Health Education as needed.
Installation of new HMO groups
Patient Resources works with the clinics, patients, and HMO’s to install new groups within our organization.
Primary Care Physician Panel Status Liaison
Patient Resources is the liaison for panel status changes. We work in conjunction with the appropriate parties to ensure proper communication regarding physician panel changes.
Patient Resources offers a variety of training opportunities for faculty and staff, as needed. Some of the topics we present include Patient Resources services, Advance Care Planning, and education regarding domestic violence.
