Staff Script for Obtaining an ABN from a Medicare Patient

This script is provided as a tool to assist staff in explaining the Advanced Beneficiary Notice to patients:

“We expect that Medicare will not pay for the test/service on this form. This does not mean that you should not have this test/service. Your doctor thinks it is important for you to have this test/service, but the decision to have or not have the test is up to you.

Medicare requires us to give you this form to help you decide if you want to have the test, knowing that you might have to pay for it.

Please read this form carefully and check whether or not you want to go ahead with the test. If you have any questions or do not understand the form, please let me know and I will be happy to help.”
