Referring a Patient for Clinic Appointment
Prior to scheduling a consultation appointment for your patient at the Wisconsin Sleep Clinic, we require the following:
- A recent clinic note from the primary care physician describing the clinical issue pertaining to the patients sleep problem and any attempted treatments
- A recent (less than six months) physical exam
After gathering and reviewing the necessary information, Wisconsin Sleep will assign the request to the appropriate sleep specialist and contact the patient within two business days to schedule an appointment.
Patient Self Referral for Clinic Appointment
Potential patients can go to the Wisconsin Sleep website and take the Sleep Apnea Screening Questionnaire. If the potential patient is suspected of sleep apnea after submitting their questionnaire, they will be able to request an appointment.
If the questionnaire is positive for sleep apnea the patients information submits to a sleep physician for triage, after which a call is made to the patient to schedule a consult.
Potential patients also have the option of calling Wisconsin Sleep.A Wisconsin Sleep staff member will do the sleep apnea evaluation over the phone.
