Home Care Services - Departments & Programs, UW Health

UW Hospital and Clinics provides and coordinates a full array of services to support patients after discharge. Home care services are planned while the patient is still in the hospital and provide quality care to patients who require special services and technology in the home. Services include home health (skilled nursing, home health aide, and therapy), home infusion, home respiratory equipment and services, as well as home medical equipment and supplies. All services provide emergency on-call support 24 hours per day, seven days per week.

How do I refer a patient for home equipment (HME) or supplies?

Patients need to be informed that they have a choice of vendors. If they choose to have UW Health Home Care supply their needs, contact (608) 203-2273. Always call this number before sending a patient to Home Care to be sure staff and supplies are available when the patient arrives. The UW Health Home Care office is at 2030 Pinehurst Drive in the Middleton Business Park. Orders can be faxed to (608) 203-2240.

Will insurance cover home health equipment and supplies?

Insurance will usually cover a portion of the equipment and supplies, but there are many rules that govern qualifications for coverage. Please contact UW Health Home Care at (608) 203-2273, and we will be happy to work with providers, patients and the insurance companies to establish coverage.
