HIM-Document Imaging is responsible for the document imaging (scanning) of all documentation received through multiple mediums into the UWHC patient's electronic medical record (EMR). To ensure the EMR is accurate and consistent it must meet State, Federal regulations and accreditation standards. Document Imaging is centralized within HIM and is not open to users within clinical areas. 

Documents scanned into the EMR must be on UWHC approved forms (excluding outside documents) and approved for scanning through the Document Imaging Committee which contains representatives from both UWHC and UWMF. All new scanning requests are to be submitted to HIM for review by the Document Imaging Committee. The request form should be utilized when submitting a request for scanning.

Upon receipt the Document Imaging Committee will review for approval, determine document type naming conventions and appropriate filing within our Health Link- EMR.
