OPEN ENROLLMENT QUICK STEPS

ξ FLEXIBLE SPENDING ACCOUNT—Employees must enroll annually in a flexible spending account or health savings
ξ QUARTZ HEALTH INSURANCE—Unity, Physicians Plus and Gundersen Health have all integrated under the Quartz
brand. Employees currently enrolled in any of these plans will automatically be move to Quartz UW or Quartz
Community and will receive new insurance cards. No paperwork is necessary unless you want to make other
ξ If you are currently enrolled in one of the following plans, you must select a new health plan during Open
Enrollment or you will not have coverage effective 1/1/2018: Anthem Blue Preferred Northeast, Arise Health Plan,
Health Tradition Health Plan, Humana Eastern and Western or UnitedHealthcare of Wisconsin.
ξ WISCONSIN RETIREMENT SYSTEM (WRS) employer and employee contributions are decreasing in 2018 from 6.8%
ξ INCOME CONTINUATION INSURANCE—ETF announced an overall ICI premium increase of 20% to go into effect
February 1, 2018. The new premiums for 2018 can be found on U-Connect.
ξ SUPPLEMENTAL LIFE INSURANCE — Premiums for employee coverage are decreasing by 20% for 2018. If you were
enrolled in this plan on or prior to 1/1/2017, and are not at the coverage maximum, you will receive an email
invitation during Open Enrollment to increase coverage without having to go through medical underwriting.
ξ OPT OUT INCENTIVE—If you are opting out of the medical benefit for the 2018 benefit year, you must submit an
application to be eligible for the Opt Out Incentive in 2018.
Submit enrollment forms through an Ask HR case or fax to ensure it is received by the
October 27 deadline. Flexible Spending, HSA and VSP enrollment must be done online.
WHAT DO I NEED TO KNOW?
As part of the approved State budget, Domestic Partner coverage will not be available in 2018. Domestic
Partners and their dependents will not be covered under any of the UWHC insurance plans beginning
January 1, 2018. Domestic Partners who are currently enrolled on your insurance plans will be covered
through December 31, 2017 and will be offered COBRA coverage for the applicable benefits. Additional
information and instructions on removing impacted individuals will be communicated through U-Connect, as
well as through a targeted email following the end of Open Enrollment.

ι Some networks are changing and/or being eliminated and may require action to enroll in a different plan or you may not
have coverage as of January 1, 2018.
ι 0% increase in employee-paid premium contributions; Increase for those paying 50% or 100% of the full premium
ι Prescription Coverage: CVS pharmacy, including Target pharmacies, are no longer in-network
SUPPLEMENTAL DELTA DENTAL INSURANCE
ι Opportunity to enroll, waive or change your current election
ι This benefit will be offered as an Open Enrollment opportunity, therefore removing the ‘late enrollee clause’ and associated
waiting period for benefit
ι If you are currently enrolled in this benefit and are in a waiting period, your remaining waiting period will be waived and
your benefits effective January 1, 2018. No action is required on your part.
ι Domestic Partners will no longer be covered by this plan in 2018.

2018 MAXIMUM ELECTIONS
Health Care Flexible Spending Account: $2600
Dependent Care Flexible Spending Account: $5000
CHANGE FOR 2018 BENEFIT YEAR:
Flexible Spending Account deductions will be taken from 24
paychecks, rather than 26 paychecks. This includes flexible
spending, parking and transportation.
HEALTH SAVINGS ACCOUNT (HSA)

2018 MAXIMUM CONTRIBUTIONS
Note: These maximums include employer contributions to the
UWHC contributes $750 individual/$1500 family.
If you are enrolled in a High Deductible, you must enroll in an
associated Health Savings Account.
