Review Employee Self-Service to see what you are currently enrolled in. Employee
Self-Service can be accessed through U-Connect > Quick Links > Employee Self-Service.
ξ If you don’t wish to make changes to your current elections, you do not need to take any action to re-
enroll. You only need to take action if you wish to make changes to your current benefit election or if
you wish to enroll in a flexible spending account for 2018. IRS regulations require new FSA elections
ξ As you are a Physician dually employed with UW Medical Foundation and UW School of Medicine and
Public Health, you do have benefits available from both organizations. This information is specific to
UW Medical Foundation benefits. Information on your UWSMPH benefits can be found at
Submit enrollment forms through an Ask HR case or fax to ensure it is received by the
October 27 deadline.
Open Enrollment provides you the opportunity to newly enroll, change plan type, add or drop an
eligible dependent, and/or cancel coverage.
As a Physician, you have benefits through UW Medical Foundation and benefits through UW School of
Medicine and Public Health. The information on this guide is specific to your UWMF benefits.

FLEXIBLE SPENDING ACCOUNTS (FSA)
UW Medical Foundation offers only the Health Care Flexible Spending to eligible physicians.
If an employee wishes to enroll in both the Health Care and Dependent Care Flexible
Spending Accounts, it may be beneficial to enroll in the SMPH plan. Eligible Physicians can
enroll in the Health Care Flexible Spending Account plan through UWMF and UWSMPH for a
maximum annual enrollment of $5200.
Enrollment must be done annually through Discovery Benefit’s website:
2018 MAXIMUM ELECTIONS
Health Care Flexible Spending Account: $2600
CHANGE FOR 2018 BENEFIT YEAR:
The ‘Grace Period’, allowing you to incur eligible expenses until March 15, 2018 for 2017 FSA
funds will be replaced with an up to $500 carryover into 2019.
This change will impact 2018 benefit year elections.
HEALTH SAVINGS ACCOUNT (HSA)
If you are enrolled in an HDHP (through your SMPH benefits) and would like to use the HSA
benefit through Discovery Benefits, you will need to enroll online through Discovery Benefit’s
