2018 UWMF OPEN ENROLLMENT GUIDE
OPEN ENROLLMENT QUICK STEPS
Review this Open Enrollment Guide to learn about your 2018 benefit options, where
to find more information and who to contact with questions!
Explore uconnect.wisc.edu/openenrollment for all the details on premiums, plan
changes, enrollment and more.
Review Employee Self-Service to see what you are currently enrolled in. Employee
Self-Service can be accessed through U-Connect > Quick Links > Employee Self-Service.
ξ If you don’t wish to make changes to your current elections, you do not need to take any action to re-
enroll. You only need to take action if you wish to make changes to your current benefit election or if
you wish to enroll in a flexible spending account for 2018. IRS regulations require new FSA elections
each year.
ξ QUARTZ HEALTH INSURANCE—Unity, Physicians Plus and Gundersen Health have all integrated under
the Quartz brand.
ξ DOMESTIC PARTNER AFFIDAVIT—Effective January 1, 2018, all employees who wish to have a Domestic
Partner on record with UWMF must complete the 2018 UWMF Domestic Partner Affidavit and provide
the necessary documentation. If you have a DP covered by an eligible benefit (ex: health, dental,
supplemental life), and do not complete this step, coverage will end on December 31, 2017 for your DP
and DP dependents. Additional information will be communicated through U-Connect, as well as
through a targeted email.
ξ SHORT TERM DISABILITY benefits will be paid out at 60% of your salary after 7 days of disability
ξ PAID PARENTAL LEAVE will increase from a Baby Day (one paid day) to one week of paid time off for
eligible employees.
Submit enrollment forms through an Ask HR case or fax to ensure it is received by the
October 27th deadline. Flexible Spending and HSA enrollment must be done online.

Open Enrollment provides you the opportunity to newly enroll, change plan type, add or drop an
eligible dependent, or cancel coverage. As a UW Medical Foundation employee, you have the
opportunity to make changes to certain benefits during this Open Enrollment period.

2018 MAXIMUM ELECTIONS
Health Care Flexible Spending Account: $2600
Dependent Care Flexible Spending Account: $5000

CHANGE FOR 2018 BENEFIT YEAR:
The ‘Grace Period’, allowing you to incur eligible
expenses until March 15, will be replaced with an
automatic $500 carryover into 2019. This change will
impact 2018 benefit year elections.

Note: If you were enrolled in an HSA with Discovery
Benefits in 2017, you do not need to re-enroll for 2018.

EMPLOYER CONTRIBUTION TOWARDS HSA:
Employee Only: $750
Family: $1500
If you are enrolled in a High Deductible Health Plan, you
must enroll in an associated Health Savings Account.

2018 MAXIMUM CONTRIBUTIONS
Single: $3450
Family: $6900

Note: These maximums include employer contributions
to the HSA.

If you would like to contribute to the HSA for 2018, you
must submit an HSA Data Collection Worksheet to
Human Resources indicating your election amount.
