Use the following chart to determine how to obtain review and signature on a document required to be signed by a UWHC official. UWHC Legal Department may authorize exceptions.

1. Purchases by UWHC

[1] These procedures apply regardless of what the document is labeled - "contract," "agreement," "memorandum of understanding," "letter of agreement," "letter of intent," "license," etc.

[2] The Signature Authority Policy (SAP) governs who may sign documents. Do not sign documents on behalf of UWHC unless you have authority pursuant to the SAP.

[3] UWHC has standard affiliation agreements that it requires to be used for students coming to UWHC.

Standard affiliation agreement - in state

The location of the school determines which agreement is used. When a school requests changes, forward the request to the UWHC Legal Department. There are already agreements governing programs from all UW campuses (except the UW School of Medicine and Public Health). All that is required for a UW System program outside the medical school is a program memorandum.

When UWHC trainees and students go elsewhere, submit the proposed agreement from the other location to UWHC Legal department for review.
