Chronic Disease Management is an approach used to ensure that patients are receiving appropriate treatment for their chronic conditions. It is also used to attenuate or prevent complications that can arise from a chronic condition and can include questionnaires, laboratory testing or procedures.

UW Health utilizes decision support tools such as Reporting Workbench Reports and Registries to identify patients with a chronic condition. Guidelines for management of these conditions are available with recommended best practices. In addition there are educational materials specific to specified chronic conditions available for both staff and patients.
