Telephone Nurse Triage
Telephone nurse triage is used in this clinic
to help answer patients’ health concerns and
questions. When you call the clinic with a
question, a computer message is sent to a
registered nurse (RN) who will review the
information you have provided. Every effort
will be made to return your call as quickly
as possible during the same business day. If
more information is needed, a registered
nurse or other medical staff will call you
back.

After Hours, Weekends, Holidays
On-Call Providers are available after hours,
weekends, and holidays for urgent
questions. Please call

Medicine Renewals
If you need a medicine prescription
renewed, we ask you to contact your
pharmacy. They will fax the request to the
provider’s office. Our goal at UWHC is to
complete prescription renewals within 2
working days, Monday through Friday. If
there is a reason that the renewal will not be
complete within 2 working days, we will
call you by phone to let you know when you
can expect the renewal. Please phone your
pharmacy to refill your prescriptions several
days before you are running out of medicine
to allow time for a renewal if needed. Call
your clinic for renewal of narcotic
prescriptions as a paper prescription and
more time will be needed.

Letters and Forms
For forms that need to be completed by your
provider, please bring form to clinic
appointment or fax form to:

Please include instructions on your fax
letting us know how you would like the
forms returned to you; such as an address
that we can mail form to or a safe fax
number. Please allow _________ days for
this to be completed.


Privacy
UWHC honors a patient’s right to
confidentiality of medical information as
provided under federal and state law. For
any patient 18 years or older, written
authorization is needed in order to speak
with anyone other than the patient. If you
would like to give permission for UWHC
staff to speak to a person other than
yourself, please call the clinic to have the
Authorization for Verbal Communication
form mailed to your home or complete the
form during your next clinic visit.


Alternatives to Calling Clinic
MyChart is a great way to keep in touch
with the clinic using a computer or smart
phone for non-urgent questions or requests.
