Questionnaires are reviewed every 3 years unless otherwise indicated by special circumstances such as licensing period requirements or validated updates to the tool. Tools are reviewed for content correctness, build appropriateness, and clinical usage.

NEW TOOL DEVELOPMENT PROCESS

To request usage of a new tool please use the “Questionnaires Request Form” link at the bottom of this page. All requests are prioritized based on content review, licensing, regulatory requirements, and collateral work. Questionnaire approval typically ranges from 1-3 months with tool build determined by analyst resources. A CCKM staff member will contact you with any questions regarding your request.

All Questionnaires must be approved by the Questionnaires workgroup before they can be used in clinical practice.

OVERSIGHT COMMITTEE

The Questionnaires workgroup meets the first Thursday of every month to review and approve tools. The workgroup contains membership from the following areas: CCKM, physicians, IS Inpatient and Ambulatory, Operations (primary care and specialty care), Legal, Nursing Informatics, HIM, MyChart, and Inpatient Nursing.
